Welcome back BHS students and parents !! We have missed you during these trying times! 

Welcome to BHS and our School Health Office. I am Nurse Amanda Durrett RN, BSN. Many of the students know me as Nurse D. I graduated from Lipscomb University in 2016 with a Bachelor of Science in Nursing. My professional practice areas include Cardiac Stepdown, Medical-Surgical, Field Nursing and Public School Nursing. I joined Rutherford County Schools (RCS) in 2018 and have been at Blackman High School since that time. 

Currently, I serve as the BHS First Responders Team Leader. In addition, I am certified in Advanced Cardiac Life Support (ACLS) and serve as an American Heart Association CPR, AED and First Aid Instructor.

Feel free to fax or have your physician's office fax any current health forms, TN Immunization Certificates, recent physicals to the fax below. Please note, all medical forms are required to be updated ANNUALLY and can not be carried over to another school year. If you need updated forms, please see the Medical Forms link to the right.

Physical Exam  
If you have a student coming from out of state, homeschool or private school, you must turn in a proof of a health physical exam for your child within 30 days of registration.  After this, your child must be excluded from school per Rutherford County policy. If your child has already had a physical within 1 calendar year, that can be submitted.
